Is there a way to identify if an app is unprovisioned from the device, since the app itself is not removed from the device.

    @Bevan Thomas, Thanks for posting in Q&A. From your description, it seems you remove the app via custom profile. If yes, you can check the device status of the profile in Intune portal to see if the policy has been applied to the device successfully. If it is failed, you can check the Event log under Applications and Services Logs > Microsoft > Windows > DeviceManagement-Enterprise-Diagnostic-Provider > Admin to see if any error exists
