Unable to create knowledge base in QnA maker portal

Hi Team,
I once created a knowledge base using QnA maker web portal (refer screenshot A) but now it shows that there is no such knowledge base that exists (refer screenshot B).

I have a free azure subscription right now with me. When i try to create a new KB provided that i already have QnA service created (refer screenshot C), post i click on Refresh it does not load the relevant options under "Azure subscription name" dropdown (refer screenshot D)
