In On-Prem Exchange, you could set email address policies to automatically configure a number of email addresses on email box creation (jdoe@conteso.com, john.doe@conteso.com, john@alternateconteso.com, ect). Is there a way to still do this in Office 365 that is Azure AD synced, with all users updated from AD?

    Only for Microsoft 365 Groups, as email address policies in Exchange Online don't support other recipient types. You can always use PowerShell though.
    The above applies to cloud-created objects though. If you are syncing from on-premises AD, any changes, including those to email addresses, need to be authored on-prem.

    @Bill Knox

    I wan to confirm with that:

    1. How do you created mailboxes in your organization?
    2. Whether there exist hybrid in your organization?

    If those Exchange online mailboxes are created in the way of "remote mailbox", you could create Email Address policy on Exchange on-premises. It will apply on those mailboxes, then sync to Exchange online side.

    If those Exchange online mailboxes are created in the way of "assign license" and there exist hybrid in your organization, you will could enable remote mailboxes for those mailbox from Exchange on-premises, then you will could manage those mailboxes from Exchange on-premise with Email address policy.

    If there doesn't exist hybrid in you organization, you just sync AD accounts to AAD, then assign license to enable those mailboxes. You will cannot add proxy email on those Exchange online automatically.

    In short, if there exist hybrid, you will could change those Exchange online mailboxes to remote mailboxes, then manage them from Exchange on-premises. There doesn't exist Email address policy for Exchange online mailbox, but in a hybrid mode you to manage them from Exchange on-premises.
