Windows powershell error on windows server 2016

Question

Whenver i am tryting to open powershell or exchange powershell. I Gives me below error.
error: vulnerable process has been exploited and is running:
Process name: C:\Windows\System32\WindowsPowerShell\v1.0\powershell.exe

note: All windows patches has been up to date

         No internet access in the server.

Which antivirus software is/was in use?

I would like to suggest you to below Troubleshooting steps.

1. Update Windows to latest version
2. run sfc /scannow from elevated command prompt or from safe mode.
3. Update your antivirus definitions to latest version and do Full system scan
4. Cleanup C:\Window\Temp and C:\WinNT\Temp folder
