Your system-assigned managed identity needs FHIR Data Writer role as described here and based on the above statement, it looks like you already have. So, please validate if FHIR destination is valid by going to Azure portal -> Destination blade of your MedTech service and ensure that FHIR server field is correctly filled in.

After validating, I suggest you create a ticket to our support team with required details mentioned below.
