Welcome to Microsoft Q&A forum.

As I understand, you are getting Extension host terminated unexpectedly 3 times within the last 5 minutes.

Could you please see what all extensions are installed in Azure Data Studio?

If it only happens for Azure Database for MySQL as I can see in the screenshot, then are you able to work on the MySQL database?

Also, I would suggest you to update ADS to the latest version if not already done and let us know if you still face the error prompt.
