Azure Migrate: Assessment questionnaires & prerequisites

Hi All,

We are planning to migrate 14 physical servers from on-premises to Azure utilizing azure migrate; could you please give a link for assessment questionnaires that we have to ask the customer and about prerequisites, how to collect the on-premises inventory and other details related to migration.
