I have had no luck getting the Management Point role of a Primary Site attached to a CAS to work correctly without the Primary Site MP component throwing up the dreaded:

Call to HttpSendRequestSync failed for port 80 with status code 500, text: Internal Server Error
Http test request failed, status code is 500, 'Internal Server Error'.

The site hierarchy is still under construction with a the following Site Servers up and running - CAS, SQL Server (HA Clustered for CAS Only), WSUS Server. The primary site was recently added but is configured to have it's own SQL server and db instance installed on the Primary Site Server itself. The appropriate Web Server and Client Auth Certs are issued by a Domain CA and enrolled on the site servers. The CAS and Primary Sites are setup for HTTPS or HTTP comms security with the Trusted Root CA cert defined and imported. We have removed and reinstalled the management point, we have checked the SMS MP Pool in IIS for False flag set on 32-Bit Apps, we have removed and re-installed IIS and finally we have started a new server but ran into same error about 1hr 40 mins later.

Basically tried all possible solution from the web, but no luck. Looking to see if I can get any sort of help/guidance to constructively troubleshoot this.

Thanks in advance.

Since you have a CAS, and therefore over 150K devices, you should contact your TAM for support. Very few people will have a CAS and community support is very limited.
