I have a machine with windows 1809, and I believed that when you run a "Fresh Start" from the Endpoint Manager console it should also automatically download and install the latest version of Windows ?

Nope. Fresh Start is just resetting device.
After the reset, based on Intune update policy, Quality & Feature updates are installed.
@andreasbright-4989, Agree with PaD, the Fresh Start device action removes any apps that are installed on a PC running Windows 10, version 1709 or later. Fresh Start helps remove pre-installed (OEM) apps that are typically installed with a new PC.

To download and install the feature update, we can use Windows Update for Business. Intune provides "Windows 10 update ring" and "Windows 10 feature update" policies to manage update. Here
