We found the following error displayed when user trying to confirm file name or rename file (see the error message prompt if users not have delete right. (you can update summary field but file name cannot be changed after upload)):
"Access denied. you do not have permission to perform this action or access this resource".
If user has a permission to the document, it's probably locked for 15 minutes.
User could try rename it after 15 minutes.
Verify also that the document is not open with another user.

For workaround, we have rename the file before upload (i.e. case ref. number), after update summary and click OK. we will see a same error message but title should be updated.
Unfortunately, this problem has been in SharePoint environments for years (Online and On-Premises).
15 minutes is the default time.
I think the easiest way is to rename file before the upload the file as you have done.

the fact that much of our behavior is conditioned by window 10 and IE 11 & Chrome have this problem. when i use the window 7 , IE 9 & Chrome without the problem.
Is it related or not? we need to know more detail why the problem is occurring now (in window 10 and IE 11 & Chrome).
