Is it just for one user or for several users?
It is possible that this user is using the Primary Refresh Token with the MFA claim and not getting prompted since the request is already satisfied (as described here).
Otherwise, the Azure AD sign-in logs should give you a good clue as to what is going on. You can search for the entries that correspond with the activity you have mentioned and see if conditional access policies are being applied or being skipped.
