Please help excatly what is the cause of this slow query duration, it is very badly impacting the performance.
I have checked the RAM is also enough 32GB

Whether the entire SharePoint server goes extremely slow or it is slow to perform a specific operation?

Whether SharePoint has any list exceeds the view threshold or adds some custom codes and web parts?

Whether have high CPU or memory use in the SharePoint server?

You could try to install service pack 2 and latest CU to improve the site performance.

the entire site is going slow, like if I am performing an operation and it when into loading then the other active users also face the loading issue at that time.

For Sharepoint list we have already increased the threshold and Yes we have custom codes and web parts.

High CPU or memory use - No

Yes we are planning to install SP 2 and latest CU, but I want to know that will it be really helpful or effective in improving the site performance? Or I will have to look for an alternative solution also on the other side?

In my opinion, custom codes and web parts may be the most important reason.

Installing service pack 2 and latest CU may not improve performance, but this can be an attempt. Or you could try to extend the RAM.

And I suggest you open a ticket with Microsoft for further help.
