I believe the definition of a "set" in this scenario is referring to the instances of a user forest. It can be read as - you're charge about $109.50 per month or per set of instances of your User Forest.
-A User Forest Instance consists of 2 domain controllers for high availability, spread across 2 availability zones (if available in region).
Based off the above definition, it looks like the documentation is classifying the 2 domain controllers within that instance of a User Forest a set. Because - by default, a managed domain is created as a user forest, and you can only create a single managed domain serviced by Azure AD Domain Services for a single Azure AD directory.
I completely agree that this can be confusing, and I'll reach out to my team to confirm if this definition of "set" is correct. I'll provide an update as soon as possible.

I was able to confirm my answer above.
When referring to a "set", it's the User Forest Instance of 2 domain controllers for high availability, spread across 2 availability zones (if available in region).
