SQL Server move from on-premise to cloud

My company has finished moving an entire SQL Server instance from an on-premise server in the building to the cloud on AWS.
The license of that SQL Server instance was purchased with the server itself from Dell and it was purchased with software assurance.

I thought I read somewhere several months ago that there's a form that you must fill out and file with Microsoft after you perform a SQL Server move from on-premise to the cloud. Am I mistaken?

Are there any concerns I should be aware of about moving a SQL Server instance from on-premise to cloud?

This may be more about licensing.

You have SQL Server licenses only on paper, nowhere else. So it doesn't matter where SQL Server is running, as long as you have the license papers.

I found the documentation about this "License Verification" form:

Step #4, right above the footer.

This applies, right?
I got to fill that form out and send it in to Microsoft?

Anyone done this before?
