Azure AD Enterprise Application request form

I am an administrator for Azure Active Directory, I have been asked by my supervisor to create a request form for people to use when they request us to activate and configure an application in Azure. Does anyone know of good location I can find such a template or insight into how to build such a request form?

The end result of the form is to have other engineers and admins use it to submit tickets with it attached so it can be reviewed vetted etc.

I am trying to see how other companies and admins are approaching such requests and to get thoughts, ideas, resources, etc.

    There isn't an example out-of-the-box that I know of that Microsoft provides for this, but you could search for some similar applications on Github.

    The Power Platform has a template for requesting the creation of a Team on Microsoft Teams, and this has a lot of fields that might also be useful in your scenario (business justification, scope, stakeholders, etc).
