showing the table / DB feed on the side, when catagory is selected in the new discussion community sharepoint template

Hi ,

I have a requirement in our community site template form when a new discussion is added, it opens a form as shown below. My requirement is, on selecting a specific category, it also shows up a table or info from a table about the category ( a table somewhere which has info corresponding to each category) as shown below. Any idea how to achieve that? Pic 3 is the concept.

Per my test, you can refer to following code to realize autofill a textboard if you select another option.

Can you tell me how and where to insert that code and what will be the result look like?

Just to confirm I don't want to change anything in Category. Just want to pull the information from table or list, when a specific category is selected.

for example, In my second pic above, when I select a General category, I want some information to show up in the form beside body field which will be in reference to selected category. By default, there is nothing beside the new discussion form field. Can we show any field there?

You can insert the script by click edit page and add a script editor in the page like following

I have added the script and i see a box on main page but nothing in the form. I want to have a field added in the form which shows up and you select new discussion. Sorry If my question is not be clear to you. please see the below pics. let me know if you need more clarification on my requirement.

I want a field beside body in the form (this forms open when you click new discussion). how can we put a text field in the form

@RaytheonXie_MSFT I was able to get another script and tested it on F12 using chrome (pic 2 attached) and its working on front tend but when i tried to insert it using sharepoint designer, the form is not showing any changes. Any idea, how to insert that script to the form?

I have attached the script after pic 1 and please see the pics below:

You can follow the document to insert your js code to edit form.

I would recommend you to upload js file to Site Assets library. Refer the file from Site Assets or any other library. Make sure this link it not blocked by SharePoint or your network. Then add a Content Editor or Script Editor web part for inserting the code.

@RaytheonXie_MSFT I tried the link and getting following response (below) which seems like SP designer is not allowing creation of new form, but that link was very informative. So I am trying to edit the form using script.
For that I upload the script in a style library and calling it from there. I didn't get any errors, but i also did not see any changes in the form though, but i know that that script has no errors because i tested it on front end using chrome console. can you confirm an important thing here if we can actually edit exiting forms using scripts? thanks

I am all for it but the question is how to do it for form. I can use script editor on page but cant find a way to edit form. Can you show some screen shots, how you have done it?
