We have 3 management server (A,B,C)
This alert is logged for the Computer "C" .
We have dedicated A(primary), B(secondary) Management Servers for monitoring agents.
Please correct me if iam wrong, you are suggesting us to uncheck the below one in the screenshot ?

We have checked the link that you ha referred , nut in case we are able to see the reports in the reports tab(Not blank)
SSRS web URL also opening for us with out any issue.

ACS collector server and ACS database both are online, no issues with them.
This issue happening only with few servers for which we have enabled ACS forwarder.For the other servers this functionality is working as expected.
We didn't see any alerts in the console related to ACS collector or ACS DB failure.
ACS collector service is running fine on the collector server.
We are able to see the connected/disconnected event id's in the ACS collector server ops manager event logs for the servers connected successfuly.

Do i need to celar cache on all the SCOM Management servers and restart required on all ?
Also after clearing cachce on agent machines also do i need to restart.
Can we clear cache for agent installed servers and Management servers from SCOM Montiroing pane ->Task status view - clear cache or do we need to delete the Health state folder.

\Program Files\Microsoft System Center\Operations Manager\Server\Health Service State
Please confirm on this.

Thank you for the update.
I was checking the performnce graph from Monitoring -> windows computer-> selecting win 2012 server - > right click prop->Performance view- selecting the required rule.
Can you pleae let me know where you want me to try Reset to default option in my personalise view.
Clear chache on the Win server 2012 R2 Clinet computer where agent is installed?

Thank you for providing the query. We have pulled the the report using query and compared it witht the performance report. We see there are data in the report we have pulled but in the graph it is showing blank. Attaching the screenot for youir reference.
