Updates the index <c>AccountCodeIdx</c> in the table <c>ShipCarrierCompanyAccounts</c> not to allow duplicate records.

After removing duplicate records violating the uniqeness of the index, the index AccountCodeIdx is reset not to allow duplicate records.
