Business value
Additional functionality will be added to the Care Plan Development tool that will include the ability to enter templatized care plans and wellness initiative goals and activities.
Feature details
Ability to create and update existing care plans directly within the Care Plan Development tool. Also expanded functionality to provide the following:
- Add care plan templates and track activities and goals associated with the care plan.
- Expanded functionality to collaborate with the care team associated with the care plan.
- Ability to associate wellness information within the care plan.
- Integration into patient outreach solution to provide proactive outreach around care plan if needed.
- Automatic updates to care plan based on care gaps that have been identified through Customer Insights.
