After you conduct the Gap solution design workshop and review the findings, identify items as risks and issues, and their associated action items for mitigation and resolution, manage them to completion as part of the overall engagement.
Issues that are identified often affect the ability to successfully go live. Resolve these issues before the go-live readiness assessment and the deployment of a production environment.
You can manage risks and their associated recommended mitigation according to the overall strategy of the project. But they'll be monitored throughout the project. Risks that aren't mitigated might be realized later as issues that could also affect the go live.
